Anthem Patient Eligibility Criteria
  • Age 4–13 years old*
  • Diagnosis of anisometropic, small-angle strabismic (≤10 prism diopters), or mixed-mechanism amblyopia
  • 6-month trial of patching or atropine (or documented contraindication/allergy/intolerance)
  • Myopia not greater than -6.00 D spherical equivalent in either eye, if present
  • No history of light-induced seizures and able to complete 60 minutes of daily treatment

Step 1 — You Prescribe

*If patient’s medical benefit information is missing from the prescription, CoAssist Pharmacy will reach out to patient directly to request this information prior to step 2. CoAssist can be reached at 855-382-2533 with any questions.

Step 2 — We Verify

If medical benefits indicate Anthem coverage, Pentec Health will contact your office via fax or email to confirm eligibility criteria (see what to expect below). Pentec then creates, manages, and submits the Prior Authorization on your behalf. Pentec can be reached at 866-751-1077 with any questions.

Step 3 — Patient Starts Treatment

Pentec works with your patient to get them started on treatment once coverage is approved. Pentec will keep your patient updated via secure text every two weeks throughout the process.

If your patient hasn't received a text from CoAssist confirming a successful transfer to Pentec—or a request for additional information—within 3 business days, families can contact CoAssist directly to check on their prescription status at (855) 382-2533.
